A SOAP note is a standardized method of documenting patient encounters used in medical, healthcare, and counseling settings. It consists of four components: Subjective, Objective, Assessment, and Plan, providing a structured format for healthcare professionals to record patient information, clinical observations, diagnostic assessments, and treatment plans.
